Triggers of perioperative myocardial ischaemia and infarction.

چکیده
Cardiac complications are a major cause of morbidity and mortality after non-cardiac surgery. 64 However, the exact nature of perioperative myocardial injury remains elusive and an area of continued debate and controversy. 64 If triggers of perioperative myocardial ischaemic events could be identi®ed, appropriately taken preventive measures might improve perioperative cardiac outcome. Identi®cation of possible triggers of perioperative myocardial ischaemia and infarction requires knowledge of their incidence, diagnosis and characteristics; of the pathophysiology of acute coronary syndromes (ACS) in the nonsurgical setting; and of factors and interventions that have been shown to affect perioperative myocardial injury. The following review will address these areas before attempting to de®ne the possible mechanisms and triggers of perioperative myocardial ischaemia and infarction.

Perioperative myocardial ischaemia.
The goal of anaesthetic management in patients with coronary artery disease (CAD) is the prevention of perioperative myocardial ischaemia. This approach is based on the assumption that sustained intraoperative ischaemia may precipitate postoperative myocardial infarction (MI) with its attendant complications. This presentation will review available information regarding perioperative ischaemia,...
متن کاملEmotional Triggers of Acute Myocardial Infarction
Introduction: Cardiovascular diseases are the leading causes of death around the world. Identification of triggers that lead to acute coronary events in ischemic heart diseases and their prevention can reduce the complications of myocardial infarction. Objective: The purpose of this study is to determine emotional triggers in patients with acute Myocardial Infarction (MI). Materials and Metho...

One hundred consecutive patients undergoing coronary artery bypass surgery were randomly allocated to a preoperative (24 h) intravenous infusion of isosorbide dinitrate (1.5-15 mg/hr) (50 patients) or to placebo (50 patients). The characteristics of the two groups were similar. Evidence of acute myocardial ischaemia was sought by continuous electrocardiographic Holter recordings and acute myoca...

Myocardial ischemia (MI) resulting in infarction is an important cause of mortality and morbidity worldwide. Acute ischaemia rapidly impairs myocardial contractile function. Myocardial dysfunction persisting for several hours after transient non-lethal ischaemia, eventually resulting in full functional recovery is termed as myocardial stunning. Hibernation is now thought to be...
متن کاملAnaesthesia and myocardial ischaemia/reperfusion injury
Anaesthetists are confronted on a daily basis with patients with coronary artery disease, myocardial ischaemia, or both during the perioperative period. Therefore, prevention and ultimately adequate therapy of perioperative myocardial ischaemia and its consequences are the major challenges in current anaesthetic practice. This review will focus on the translation of the laboratory evidence of a...
